My private pay rate is $150 for a 75-minute intake. All NEW clients will need to schedule an intake so that I can better understand the client’s needs and develop a treatment plan that we can work through together. All subsequent individual sessions

are $150.

I am currently in-network with several major health insurance providers in the state of Texas: UnitedHealthcare, Blue Cross Blue Shield, Aetna, Cigna Healthcare, Oscar Health, and Oxford Health. I would ask that you please contact me to verify your coverage as my provider list can change from time to time.

In some cases, insurance companies provide out-of-network benefits. If this is your case, I can provide you with an invoice that you can submit to your insurance provider for reimbursement for sessions. You can find out more by contacting your insurance company and asking about your mental health out-of-network benefits.

I suggest calling the customer service number on the back of your insurance card.

Here are a few helpful questions to ask your insurance provider about your out-of-

network benefits:

• Do I have out-of-network benefits? If so, how much does my policy cover if I see an out-of-network provider?
• Does my current policy have a deductible? What is the amount, how much has been met this year, and when does my policy renew?
• Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
• Will my health insurance reimburse me for 60-minute sessions (CPT 90837) or only 45-minute sessions (CPT 90834)?
